چکیده

Otitis media (OM) is a common pathology in peadiatric cases. Serious and life threatening complications including petrositis, epidural abscess, subdural abscess, cerebral abscess and meningitis, may be seen with OM. However they become rarely seen nowadays due to the improvement of antimicrobial therapy. Beside this, they may be silent but with dangerous evolution . In 1904, Giuseppe Gradenigo described the syndrome, which carries his name and consists of the following triad: acute and chronic suppuration of the ear; paresis or paralysis of the ipsilateral abducent nerve; and ipsilateral trigeminal neuralgia due to petrous apicitis . The atypical clinical features also need special attention. The present case report describes the clinical management of a child with incomplete Gardenigo syndrome consisting of petrous apicitis and abducent nerve paralysis without otitis media symptoms and trigeminal neuralgia.

Gradenigo’s Syndrome in a Patient with Chronic Suppurative Otitis Media, Petrous Apicitis, and Meningitis

BACKGROUND Gradenigo's syndrome includes the triad of suppurative otitis media, ipsilateral sixth (abducens) cranial nerve palsy and facial pain in the distribution of the fifth (trigeminal) cranial nerve. Gradenigo's syndrome is rare, and the diagnosis is easily overlooked. This case is the first to report Gradenigo's syndrome presenting with meningitis on a background of chronic suppurative o...

Petrositis With Bilateral Abducens Nerve Palsies complicated by Acute Otitis Media

Petrous apicitis is a rare but fatal complication of otitis media. An infection within the middle ear can extend within the temporal bone into the air cells of the petrous apex. With only the thin dura mater separating the trigeminal ganglion and the 6th cranial nerve from the bony petrous apex, they are vulnerable to inflammatory processes, resulting in deep facial pain, lateral rectus muscle ...

Gradenigo’s syndrome secondary to chronic otitis media on a background of previous radical mastoidectomy: a case report

INTRODUCTION Gradenigo's syndrome is nowadays a rare condition characterized by a triad of otorrhea, facial pain with trigeminal nerve involvement and abducens nerve palsy. Most cases are caused by medial extension of acute otitis media into a pneumatized petrous apex and surgical drainage is usually the treatment of choice. We present a case highlighting the pathological mechanism of this dise...

Acute Otitis Media-Induced Gradenigo Syndrome, a Dramatic Response to Intravenous Antibiotic

Introduction: Petrositis is a rare but severe complication of acute otitis media and mastoiditis. Despite efficient antibiotic therapy, there are still reports of both intratemporal and intracranial complications of otitis media with the potential risk of high morbidity and mortality. Petrositis has traditionally been treated with surgery, but recent advances in imaging, with improved antibioti...
